ALPHA FOOTBALL ACADEMY RELEASE OF LIABILITY

I, being the parent/guardian of the aforementioned student(s).

 

I acknowledge that the above information is factually correct. I therefore agree to them participating in the activities of ALPHA FOOTBALL ACADEMY held on all locations, and the use of all their sports equipment.

I agree that I will not hold the coaches of the academy or the site liable for any losses, liabilities and damages, injury or death suffered or incurred by my child whilst taking part in any of the activities run by ALPHA FOOTBALL ACADEMY.

ALPHA FOOTBALL ACADEMY MEDIA CONSENT

Photographs and / or videos may be taken of my child taking part in sporting activities for promotion of the club and displayed on the website, social media, press release, articles, and in newsletters in accordance with child safeguarding guidelines. 

ALPHA FOOTBALL ACADEMY DATA PROTECTION AND CONFIDENTIALITY POLICY

ALPHA FOOTBALL ACADEMY will collect and use/process your personal information in the manner described below:

ALPHA FOOTBALL ACADEMY does not sell or rent personal information collected from you to third parties.

You acknowledge that your personal information may be transmitted and used/processed by ALPHA FOOTBALL ACADEMY and its authorized representatives entities outside of your home country/jurisdiction and explicitly specifically consent that your personal information can be transmitted stored and processed in such third countries for the purpose specified in paragraph above.

FEE STRUCTURE

Standard Package

Single - OMR 30/- per month (2 sessions per week)

Siblings (1+1) - OMR 55/- per month (2 sessions per week)

Siblings (1+1+1) - OMR 80/- per month (2 sessions per week)

Pro Package

Single - OMR 45/- per month (3 sessions per week)

Siblings (1+1) - OMR 85/- per month (3 sessions per week)

Siblings (1+1+1) - OMR 125/- per month (3 sessions per week)

Elite Package

Single - OMR 50/- per month (4 sessions per week)

Siblings (1+1) - OMR 95/- (4 sessions per week)

Siblings (1+1+1) - OMR 140/- (4 sessions per week)

MANDATORY CLAUSE

Fee should be paid before the 10th of each month.

Classes are non-refundable (including weather conditions, public holidays/festivals).

No reimbursement on fee.

Updated season kit should be purchased.
